If you’ve already had an appointment with us after February 2026, completed the intake form, and submitted medical cards for all household members, you may go directly to the Scheduling section to book your appointment.

✅ Step 1: Complete the Intake Form Below
Fill out the form with your household details and provide consent at the end.
⏳ We recommend completing this as soon as possible.
Otherwise, part of your appointment time will be used to collect missing information for you and your household, which may limit the housing and other services we can review with you.
📅 Step 2: Schedule Your Appointment
Follow these steps:
1️⃣ Complete the intake form below or visit:

2️⃣ After submitting the form, you will be automatically directed to the scheduling page
3️⃣ For Service, select:
👉 “In-Person or Virtual”
4️⃣ For Worker, select:
👉 The staff member and confirm whether they offer in-person or virtual services
5️⃣ Select your preferred date and time, then click “Schedule Appointment” ✅
6️⃣ You will be directed to the consent page — sign and click “Finish” 🎉
⚠️ Important: If you reach the consent/sign page but do not click “Finish,” your process is incomplete and your appointment will not be scheduled.

Group Appointment Requests
If you are a case manager, hospital, clinic, shelter, or other similar entity seeking to bring your clients to receive our services, please complete the below form to book a one-day appointment.
